STEP 1: 15 pts.
SECTION I: DEMOGRAPHIC DATA
This section describes in detail the demographic data of your community. This is the overall demographic data. The topics include the following:

SECTION II: PREVIOUS COMMMUNITY STUDIES
Identify at least 3 Community Needs Assessments. You will use the Focus Groups Data from each assessment to create this section. You will use the general topic areas from the focus groups to assist you with creating your service gaps analysis for your specific population. We will be reviewing how to create a data analysis from the focus groups using examples of community needs assessments. This section will be broken down into three parts that you are able to take each part and add to this section.

- 1.)Locate and Review 3 examples of community assessments and complete the community assessment form that will be provided in class.
- 2.)Create a Community Readiness analysis using the focus groups data and the summaries provided the community assessment and the Round Robin Exercise that will be conducted during class.
- 3.)Create a Root Cause Analysis outline using the all of the information provided in number 2.

SECTION III: Root Cause Analysis Creating a Root Cause Analysis for the identified target population.
This assignment has 3 parts. First you have to identify your target population and the need for the service gaps analysis for that specific population, then you need to conduct preliminary research that will apply to your service gaps analysis, next you develop your Root Cause Analysis using current statistics and data, then you will share your findings to the class and instructor for feedback. You do this during class by describing the name of your project, your documented need, and the information you found to support your chosen population for the service gaps analysis. In this manner all students can review the intended projects of other students and give feedback.

First you must identify your target population and the need for the service gaps analysis for that specific population, then you need to conduct preliminary research that will apply to your service gaps analysis.Based upon reviews from local community assessments, demographics, and anecdotal accounts from community meetings, determine what gaps in service exist in your community for the population of your choice. Your instructor will help identify sources of a needs assessment or community plan that outlines such recent research. Resources such as United Way, local coalitions, hospital assessments, Community Health Improvement Plans, and demographics can assist you.If no local community needs assessment has been formally conducted, choose a service that you have good reason to believe is needed in your community.You can choose an existing organization that you feel needs additional resources to operate adequately.
Write a brief summary (1 page) about your target population and the need for a service gaps analysis. List the resources that you will use to research the gaps in services.
Develop your Root Cause Analysis What type of target population you want to provide a service gaps analysis. Create a Root Cause Analysis to show how the service gaps analysis can illustrate gaps in service for vulnerable populations like aging adults, runaway youth, and chronically unemployed and impoverished adults.
You will be given a template for the Root Cause Analysis. Submit the Root Cause Analysis to Chalk and Wire. Due the end of Week 5 on Sunday.
SECTION IV: ASSESSING COMMUNITY READINESS
For this section we will be reviewing social marketing and community readiness.We be reviewing the data through the lens of social marketing and applying examples of community readiness models and the Network Episode Model to complete this section.This part will be conducted by applying the knowledge you gained from the focus groups that was conducted in the focus groups from the community assessments that you reviewed and anecdotal accounts from community meetings, newspaper articles, and shared experiences.During class, we will review the models and demonstrate how you would apply these models during focus groups, round robin technique conducted within the community, and town hall meetings.
For the purposes of this assignment, you will NOT conduct a live assessment but review the models and apply the models with the secondary data and anecdotal accounts.You will simply explain how the models could be applied in real time and you applied them using secondary data analysis to identify community readiness.
